Online Group Workshop/Program Consent Information

Online workshop service

As part of providing an online workshop/program service to you, Psychology and Play Therapy Australia Pty Ltd will need to collect and record personal information from you that is relevant to your situation. Please let us know should you want a copy of our Management of Personal Information Policy which explains how we look after your information.

 

Confidentiality

Personal information gathered by our practice during the provision of the online group workshop.program service will remain confidential and secure except where:

1)   It is subpoenaed by a court; or

2)   Failure to disclose the information would place you or another person at serious risk to life, health or safety; or

3)   Your prior approval has been obtained to:

a)    provide a written report to another professional or agency. e.g. a GP; or

b)    discuss the material with another person, e.g. a employer or health provider; or

4)   You would reasonably expect your personal information to be disclosed to another professional or agency (e.g. your GP) and disclosure of your personal information to that third party is for a purpose which is directly related to the primary purpose for which your personal information was collected; or

5)   Disclosure is otherwise required or authorised by law (e.g. child safety mandatory reporting).

Partaking in an online group workshop/program, you will interact with other individuals.  You and/or the group members may choose to share personal information. It is fundamental to our group environment that each individual and their shared information be treated with confidentiality and respect, and it is your responsibility to maintain this. By consenting to participate in the online group workshop/program, you agree to maintain the confidentiality of all personal information discussed within the group.

 

Provision of an online service

The current service is provided by videoconferencing over the online ZOOM platform. The privacy of any form of communication via the internet is potentially vulnerable and limited by the security of the technology used. To support the security of your personal information this practice uses a licensed ZOOM account, which is compliant with the Australian standards for online security and encryption.

You are responsible for the costs associated with setting up the technology needed so you can access the online group workshop/program services. Psychology and Play Therapy Australia Pty Ltd will be responsible for the cost of the call to you and the costs associated with the platform ZOOM used to conduct online services.

To access the workshop sessions, you will need access to a quiet, private space; and the appropriate device, i.e. smartphone, laptop, iPad, computer, with a camera, microphone and speakers; and a reliable broadband internet connection.

  

Limitations of an online service

The workshop may be subject to limitations such as an unstable network connection which may affect the quality of the psychology session. 

  

Recording of the online group workshop/program

Workshop sessions will be recorded for Quality Assurance purposes by Psychology and Play Therapy Australia Pty Ltd.

 

Unable to Attend

The online group workshop/program fee is inclusive of all stated sessions.  No refunds or adjustments will be made to you by Psychology and Play Therapy Australia Pty Ltd should you be unable to attend a session.

 At the commencement of the online group workshop/program, group members will be consulted about and the possibility of consenting for group members to have access to a session recording in the event they may be unable to attend a session.  The group must unanimously consent to this for a group member to receive access to the recording.   

Psychology and Play Therapy Australia Pty Ltd reserves the right to reschedule workshop sessions at any time, without compensation, if it is deemed necessary due to staff illness or other unforeseen circumstances.

 

Withdrawal

There are many reasons why clients choose not to continue with an online group workshop/program.  If you wish to withdraw from the online group workshop/program, please feel free to discuss this with us. You will not be prejudiced. Your presence in any prior workshop will still be recorded and may be distributed to other group members where prior consent has been given, however no recording from your withdrawal date will include you or any shared information.

No refunds or adjustments will be made to you by Psychology and Play Therapy Australia Pty Ltd should you withdraw from the online group workshop/program.
